Physioga with Fiona is GDPR compliant and is registered with the Information Commissioners Office on the “Register of Data Controllers” as required by the Data Protection Act 1998. All data held is processed in accordance with GDPR. Physioga with Fiona will remain registered with the Information Commissioners Office.

Physioga with Fiona collects and process personal information that is voluntarily provided via the website when you express an interest in obtaining services, information or to participate in activities.

- Identifying information related to you (name, address, phone, email) if you choose to access the service.

- Further detailed information via a PAR-Q Questionnaire and waiver prior to attending a yoga class. This gathers information related to your medical history, medication and emergency contact for health and safety purposes. 

 

What happens to the information gathered:

Your information is required to understand your needs and provide you with the best possible service. The information gathered also ensures you an safely participate. Information is required to:

- Maintain up to date records

- If consulting Physioga with Fiona for Physiotherapy services, there is a legal requirement to maintain up to date relevant, accurate and timely notes following all contacts with clients, carers or professionals involved with your care. This may include electronic or paper records and photographs with your consent. There may also be a need to contact another health care professional to discuss your case for onward referral. Your consent will be gained prior to onward referral.

Where necessary, information may be acquired from other health and social care professionals and other relevant agencies. For example, your GP or social worker. However, your consent would always be obtained first. 

 

Your consent will always be obtained before sharing information with any third parties accept where there is a justifiable reason for disclosure. For example, if there is a significant risk to yourself or others we may be obliged under our duty of care to inform a person in a position to ensure safety of you or those working with you.

All third parties are required to respect the security of your personal data and to treat it in accordance with the law. Third-party service providers are not permitted to use your personal data for their own purposes and they are only permitted to process your personal data for specified purposes and in accordance with instruction.

 

Use of Email

E-mail is generally the preferred method for communication. Confidential documents attached to any emails will be password protected. Whilst all efforts are made to keep emails secure, Physioga with Fiona is unable to take responsibility for security breaches. If such a security breach were to become apparent, then you will be informed of this. If you would prefer to be contacted using other methods such as by phone or using the postal service then please discuss this with Physioga with Fiona.

Client information will be stored under password protection. Your personal information will be stored only for as long as you are an active participant within a Physioga with Fiona Class. 

 

Information relating to your physiotherapy care record will be stored for 7 years. A client’s personal data and rehabilitation information is destroyed securely in line with GDPR 18 and internal policies.

Your right of access - You have the right to ask us for copies of your personal information. 

 

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
